Youth Empowered Family Navigator Project

Support from parents and advocates who’ve been there—helping families navigate the transition after treatment and feel less alone in the process.

Welcome

We are here to support you and your family during the critical transition from a Residential Treatment Facility (RTF) back to your home and community. Services can begin up to two months prior to discharge and extend for four months after, so you have sustained, comprehensive support when you need it most.

Our goals

Our primary goal is to equip you and your family with the tools, resources, and support necessary for a smooth transition.

  • Foster strong connections during and after treatment.
  • Address both immediate needs and long-term recovery goals.
  • Enhance the overall well-being and success of the families we work with.
  • Help families identify resources and supports that fit their unique situation.

How the Family Navigator Program helps

Who supports my family?

A Peer who understands your challenges, has been through similar experiences, and offers real-world guidance as you move through this transition.

What does peer support include?

  • Support grounded in lived experience and understanding.
  • Practical strategies developed together based on your family’s needs.
  • Emotional support during uncertain or stressful moments.
  • Connections to helpful resources, services, and community supports.

Program length & cost

This support is provided at no cost to you through grant funding. Services typically begin up to two months before discharge from the RTF and continue for four months after you return home.

How to enroll

  1. Contact us at ympa@youthmovepa.org.
  2. Meet your Peer Coordinator and share your goals and concerns.
  3. Receive guidance and support tailored to your family’s needs.
